EMT & Paramedic Courses Registration Form

COURSE REGISTRATION

Welcome to PERCOM Admissions!

All online courses with PERCOMOnline Inc. are open enrollment, which means that there is not a set date that classes start. The start date is different for each student and is based on when they complete the registration process.

While there are not set start dates, there is a limit to how long a student has to complete registration and enroll. Please read the bullets below for our registration/enrollment policy.

  • The time allowed for registration and enrollment is 10 business days from the delivery date the Registration Link email.
  • All registration steps must be complete within the allowed time frame. If they are not, registration will be  canceled and a refund processed per our policy.
    Per policy, the application fee portion will not be refunded.
  • New students whose background checks are still processing after 10 days and have submitted all other  registration steps, will be exempt from this policy.
  • New students who have only submitted a background check and have not completed the other steps are subject to this policy.

NOTE! It is HIGHLY recommended that you, the student, use a Gmail account during your enrollment at PERCOMOnline!

Users of some email servers(ex. – private servers, some government servers, Yahoo, Outlook, and iCloud) may experience delivery issues with our notification emails.

PROGRAM EXPECTATIONS

Each Program in our School has deadlines by which students are expected to complete didactic and/or the overall Class or Program.  They are:
 
EMT – 3 months for completing through the final exam and the other 3 months to complete the EMT Exit Session (skills) and all rotations.  Total 6 months.
 
P1/AEMT – 5 months to complete through the final exam and the other 4 months is to complete Skills Practice and Testing Sessions and AEMT level rotations (if certifying as NREMT-Advanced).  Total 9 months.
 
P2 – 5 months to complete through the Paramedic final exam and 6 months to complete any remaining Skills Practice and Testing Sessions and all rotations through Final Field Internship.  The total for this segment is 11 months.
 
TOTAL time by which students are expected to be completed with the entire Paramedic Program is 20 months. P1 students are expected to complete that segment within 9  months so as to stay on track to be able to complete the entire program within the time allotted.  While students MAY complete faster than the designated deadlines, exceeding deadlines will cost the student extension fees of $100 per month plus processing and must be REQUESTED by email of the Program Director.  Extensions are not guaranteed and will be granted on a case by case basis.
 

PROFESSIONAL BEHAVIOR

 
Students will be monitored and assessed regarding overall behavior, interactions, appearance, and overall student presentation and performance at various stages in the program.  Applicants should read (and considering printing a copy of) the linked document and assessment form in its entirety.  Be aware, students may be given a mandatory counseling plan or even removed from the Program for not successfully meeting the standards outlined in this document and assessment form and/or the Student Handbook, which is also linked here for you to read (and print for future reference).

How Do I Register?
 

Registration is a 4 part process:

  • STEP 1. Background Check Signup
  • STEP 2. EMSTesting.com signup & course request
  • STEP 3. Complete the Course Entrance Paperwork
  • STEP 4. Student Information/Document Submission via the form below.

When filling out STEP 4. Course Registration Form, you will need the following information and documentation:

  1. Full Name & Email Address
  2. Mailing Address & Phone Number
  3. Copy of Government issued Identification Card
  4. Copy of American Heart Association BLS Providers Certification
  5. Copy of Initialed & Signed Course Entrance Document
  6. Copy of a Diploma or Transcript
  7. Copy of EMT certification ( For Advanced Courses )
What Can I Expect?

After you have completed registrations Steps 1-4, it will take several days for everything to clear and be processed through Admissions before you can begin your course. 

The background check typically takes anywhere from 3 to 15 days to fully process. This depends on the applicants residential history, ex. the more places you have legally lived, the longer the background check takes, or if you are an international student.

The fees for EMSTesting.com and PlatinumPlanner.com ARE NOT included in tuition! After you have been accepted into your class in EMSTesting.com, you will be prompted to enter your class purchase code that came with your textbook package or enter a credit card. This step must be completed to gain full access to the class. PlatinumPlanner.com comes later, right before you begin the Clinical portion of the program.

For Texas EMS Certification

Texas EMT Certification Process Form

Not required for enrollment into PERCOM, but will be required for certification in the state of Texas. We provide this link for students to start and complete this process while attending their course at PERCOM.

If you have anything in your criminal history that may prevent your certification as a medical professional in the state of TX, please complete the TX Pre-Screening process prior to registering here at PERCOM!!! If you do not pass the Pre-Screening, please request a refund because we will not be able to allow you to enroll.

IMPORTANT! All newly registered students MUST complete a background check through cFIRST using the signup links below in Step 1.  The TX Pre-Screening is just for state use!

STEPS 1-4

Step 1. cFirst Background Check

PERCOM uses multiple third-party institutions to complete various functions of your education. One of these is cFirstcorp.com, who runs your background check, and during clinical registration, sets up the drug screening.

Do you have a DWI/DUI, drug related charge, felonies, or violence related charge in your criminal history? Stop now, and request a refund before proceeding! Our clinical sites will not accept you, therefore we cannot!

INSTRUCTOR COURSE STUDENTS! Do NOT complete a background check! If you do, you will be charged $250 that must be paid before you receive your course completion!

Please select your background check based on your country of residence. If you do not see your country in the list, please contact PERCOM SUPPORT!

This process takes anywhere from 2 to 15 days before results are posted. Your enrollment will not be finalized until AFTER your background check has cleared, and has been verified by Admissions.

 

Step 2. EMSTesting.com Signup

Class signup at EMSTesting.com is mandatory for all students. EMSTesting is where you will be logging in to access your assessment testing, research and final exams, and where your instructor will keep track of your grades.

This signup for this requirement is completed by you, the student. You will need to visit EMSTesting.com, click the button that says “Student Signup Here“, and create your account. During that process, you will be prompted to enter a course code for your course. Use the list below to identify the correct code.

  • EMT Course: PBNN8

Step 3. Download & Sign Course Entrance Document

Each EMT, AEMT, & Paramedic student is required to download, initial, & sign either digitally or physically the Course Entrance Document associated with their course(see below).

Please click the link that matches your course registration to download the PDF form. Submit your completed form in the Registration Form in PDF format.

STEP 4. Course Registration FORM 

Please fill out and submit the following form to complete the registration steps. Due to some issues with uploading documents, there is now a file size limit on each document.

This form will only accept .pdf files! Please convert any file you have for this form into the acceptable format.

If you do not know how to convert your file to .pdf, please use the following FREE PDF converter to convert your file.

If you have to manually email in a document, again, do not send anything that is not in .pdf format. If you do, your file submission will be rejected.

ALSO! For you image-based PDFs, please reduce the file size of your images before you create your PDF. These files can be extremely large if you do not, and will exceed the file size limit of this form by itself!
